, we acknowledge that our employees are what makes us great! We are searching for a Social Worker - Hospice to provide social service activity as it relates to terminally ill patients and their families, including treatment planning and consultations with other staff members.
Hours:
40hrs Shift:
Day Shift 8am-430pm Travel:
This is a home care position.
Will require driving.
Territory includes Newport, Middletown, Portsmouth, Little Compton, Tiverton, Westport, Fall River, Assonet, Freetown and Lakeville.
Education and/or Experience Requirements:
A Master's Degree in Social Work from an accredited college or university or the equivalent knowledge or experience is required.
Licensure by the Board of Registration in Social Work in Massachusetts is required.
License in Rhode Island required within 90 days of hire.
Five (5) years of related work experience, two of which include relevant clinical experiences, and experience in Hospice, Homecare or working in the field of death and dying.
Proficient computer skills, including experience with various software, database and electronic records programs is required.
Must possess and demonstrate highly effective interpersonal, customer service and communication skills.
